Please review the following form to give consent for photographs / video's to be taken.

Multiply Music Group Ltd would like to take promotional photographs and/or video recordings of you/your child (as applicable) and advertise this media on official Multiply Music Group social channels. This content may also appear on our website and email newsletters and not limiting to printed materials produced for promotional purposes including; leaflets, posters, adverts. Multiply Music Group will not share both the name and images of the students shown in these advertisements and will not include personal e-mail or postal addresses and telephone numbers in printed publications. To comply with the Data Protection Act 1998, we need your permission before we take any photographs/film of you/your child. Please answer the questions below, then sign and date the form as indicated.

Conditions of use

  • This form is valid for five years. (Unless GDPR Stipulates that this form is to be terminated)
  • Multiply Music Group will not use these photographs/films for any other purposes than those mentioned above.
  • Multiply Music Group will not include personal details such as postal addresses and telephone numbers on our website and printed promotional materials.
  • Copyright of photographs taken will remain with the organisation named above.
